Receptionist reviews application and digitally aligning data on screen. |

Applicant submits application |

Applicant and Trade Name Owner present an agreement duly signed by both parties stating; parties in agreement, agreement term, financial agreement, location of service. |

Receptionist receives application, reviews the documents, ensures that agreement is signed by both parties, and then conveys application to department head. |

Department Head requests case file to attach pertaining application form, if file completed. |

Department Head advises Trade Name Registrar of Recommendation. |

Registrar accepts or refuses recommendations. |

If request refused, the Applicant mush be informed. |

If request granted, application is transferred to Data Entry Unit. |

Data Entry Clerk enters Data on proper Trade Name Database on Computer System, prepares, reviews, prints trade name use license certificate, and then transfers case with certificate to Trade Name Department Head. |

Department Head reviews data on license certification according to data provided on application; and signs certificate if cleared. |

13 |
Department Head opens license file by Trade Name Application, Copy of License Certificate and Receipt. |
